CONVERGENCE PRIVACY POLICY

Individual privacy and confidentiality will be maintained in all aspects of our business, including communicating with employees regarding substance use and abuse, as well as other confidential issues.

Information specific and personal to all potential employees and employees are maintained within the strictest confidentiality according to State and Federal laws to protect the privacy of the individual.

Furthermore, Applications of employment, pre-employment and employee drug test results will not be released without the written authorization of the individual other than to an applicable testing laboratory, approved CONVERGENCE staff only, legal counsel for cause, applicable State professional licensing boards, and applicable state Employment departments if a claim for unemployment insurance is submitted following termination for violation of the Drug Free Workplace Policy, or confirmed incidents violating State/Federal law.

Other information protected in the same fashion includes medical information, criminal background check results, OIG sanctions check results, sexual offenders check results and other confidential information required for employment with Convergence.

Violations of such protections will result in immediate termination of employment and prosecution to the extent allowable by law.
